Hope the training's going well Paddy and good luck at Thornbridge if you're racing (won't be there myself). Some lame excuse about seeing the inlaws at New Year.

Version 6 is available at the same address as above with the 5 replaced by 6. This version has some of the deliberate mistakes corrected and has been used for the police notification forms.
Please note that the version in Pedal Power 2012 is version 3 or before.
